Volume II (ISBN 0 10 284301 5) contains the full report of 11 cases. Although the reports cover many issues, there were three issues which were common to many situations: the support and supervision of doctors in training; communications between professionals and patients and relatives; the handling of complaints. Poor performance in these areas resulted in serious consequences for patients and their families and the NHS needs to be alert to the need for sound practice.
